ATHLETIC REQUIREMENTS
Student-athletes who are interested in participating in a sport must have all requirements met by the first day of practice. Paperwork should be handed in to the coach or athletic director. Fees should be turned in to the guidance office or paid on Skyward.
Physical/Participation Form needs to be completed once every school year (physical needs to be completed every other year)
Concussion Form needs to be completed once every school year
Code of Conduct needs to be completed once every year
Dual Participation Form must be completed each season a student athlete wishes to partake in dual sports.
Athletic fees:
$65/per high school sport, max $150/year per student
$50/per middle school sport, max $100/year per student
Single Game Admission: $3.00/student, $4.00/adult, $2.00/senior citizen (age 60+)
All-sports Season Pass: $30.00/student, $50.00/adult
***Free Senior Citizen passes (age 60+) are available for District Residents. The pass is only good for regular season home games (excludes tournaments) and can be picked up in the high school guidance office. For more information, please contact the guidance department at 262-495-7101 ext. 2202.
